Case Manager - care planning (Las Vegas, NV)

We are growing our team at Harborlight Birch Care Services Group and adding a Case Manager whose main responsibility will be care planning. You would join a large organisation of more than 1,000 people based in Las Vegas. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for family communication. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • On site in Las Vegas. • Applications are reviewed on a rolling basis.
