Case Manager - care planning (Knoxville, TN)

Summit Meadow Housing manages affordable housing communities across Knoxville, and we need a Case Manager to lead our care planning. We value patience and follow-through. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for resident services. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with resident services. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, spread across three days. • On site in Knoxville. • We reply to every applicant.
