Case Manager - care planning (Knoxville, TN)

Bright Quarry Care Services Group is a social-care provider serving older adults and people with disabilities around Knoxville. We are hiring a Case Manager to take on care planning. Teams support each other, especially on hard days. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for client records. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with client records.
