Case Manager - care planning (Knoxville, TN)

Highbridge Vale Community Partners Group is a Knoxville charity working with residents on basic needs and opportunity. We are hiring a Case Manager to take on care planning. Mission matters here, and so does looking after staff. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for volunteer shifts. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with volunteer shifts. Working arrangement • Part-time, about 16 hours a week, with flexible days. • On site in Knoxville. • We reply to every applicant.
