Case Manager - care planning (Kansas City, MO)

Silverline Oak Civic Services provides shared administrative services to local governments around Kansas City. We are hiring a Case Manager to take on care planning. We follow procedures carefully and explain them clearly. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for permits. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with permits. • Bilingual English and Spanish.
