Cedar Drift Health Group is looking for a Case Manager. The job centres on care planning for patient scheduling, and it would suit someone who likes steady, practical work. Clinicians and administrators work side by side. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for patient scheduling. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with patient scheduling. • Bilingual English and Spanish.
