Case Manager - care planning (Kansas City, MO)

Stonegate Vista Housing provides affordable homes and support services around Kansas City, and we need a Case Manager to lead our care planning. You would join a large organisation of more than 1,000 people based in Kansas City. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for lease compliance. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with lease compliance. • Bilingual English and Spanish.
