Case Manager - care planning (Jacksonville, FL)

Lumen Timber Property Management Group is a Jacksonville property manager with leasing, maintenance and resident services. We are hiring a Case Manager to take on care planning. You would join a large organisation of about 794 people based in Jacksonville. What you will do • Help with applications and appointments for resident events. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with resident events. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, spread across three days. • On site in Jacksonville, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
