Case Manager - care planning (Jacksonville, FL)

Ironwood Hollow Civic Services supports public agencies in Jacksonville with budgeting, records and programmes. We are hiring a Case Manager to take on care planning. We follow procedures carefully and explain them clearly. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for procurement. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with procurement. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, afternoons. • On site in Jacksonville. • We reply to every applicant.
