Case Manager - care planning (Jacksonville, FL)

Silverline Wilder Property Management Partners manages apartment communities around Jacksonville, and we need a Case Manager to lead our care planning. You would join a small organisation of about 45 people based in Jacksonville. What you will do • Help with applications and appointments for unit turnovers. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with unit turnovers. Working arrangement • Contract for four months, with a review at the end and a chance of extension. • On site in Jacksonville, with some flexibility after the first few months. • We reply to every applicant.
