Case Manager - care planning (Jacksonville, FL)

Silverline Thistle Learning Group is an education provider in Jacksonville serving students and families. We are hiring a Case Manager to take on care planning. You would join a small organisation of about 147 people based in Jacksonville. What you will do • Help with applications and appointments for enrolment. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with enrolment. • Bilingual English and Spanish.
