Case Manager - care planning (Jacksonville, FL)

Greenridge Thistle Early Learning is looking for a Case Manager. The job centres on care planning for classroom supplies, and it would suit someone who likes steady, practical work. You would join a small organisation of about 51 people based in Jacksonville. What you will do • Connect clients with partner agencies. • Help with applications and appointments for classroom supplies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with classroom supplies. Working arrangement • Contract for four months, with a review at the end and a chance of extension. • On site in Jacksonville, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
