Case Manager - care planning (Jackson, MS)

Highbridge Mill Care Services provides in-home and community support in Jackson. We are hiring a Case Manager to take on care planning. You would join a large organisation of more than 1,000 people based in Jackson. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for care plans. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with care plans. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in Jackson, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
