Case Manager - care planning (Jackson, MS)

We are growing our team at Redstone Rook Early Learning and adding a Case Manager whose main responsibility will be care planning. You would join a small organisation of about 40 people based in Jackson. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for meal programmes. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with meal programmes. Working arrangement • Full-time, on a regular day shift. • On site in Jackson. • We reply to every applicant.
