Case Manager - care planning (Indianapolis, IN)

We are growing our team at Clearpath Vale Property Management and adding a Case Manager whose main responsibility will be care planning. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for resident events. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with resident events. Working arrangement • Full-time, on a regular day shift. • On site in Indianapolis. • Applications are reviewed on a rolling basis.
