Case Manager - care planning (Indianapolis, IN)

Highbridge Summit Learning Group is looking for a Case Manager. The job centres on care planning for family communication, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for family communication. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Part-time, about 16 hours a week, with flexible days. • On site in Indianapolis. • Applications are reviewed on a rolling basis.
