Case Manager - care planning (Indianapolis, IN)

Ironwood Housing manages affordable housing communities across Indianapolis. We are hiring a Case Manager to take on care planning. You would join a large organisation of about 759 people based in Indianapolis. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for lease compliance. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with lease compliance. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, mornings. • On site in Indianapolis. • We reply to every applicant.
