Case Manager - care planning (Indianapolis, IN)

Highbridge Maple Housing is a nonprofit housing provider in Indianapolis, and we need a Case Manager to lead our care planning. You would join a small organisation of about 166 people based in Indianapolis. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for rent assistance. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with rent assistance. Working arrangement • Part-time, about 20 hours a week, spread across three days. • On site in Indianapolis. • We reply to every applicant.
