Case Manager - care planning (Indianapolis, IN)

Westbrook Maple Community Partners runs community programmes across Indianapolis. We are hiring a Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for grant reporting. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with grant reporting. Working arrangement • Part-time, about 28 hours a week, with flexible days. • On site in Indianapolis. • We reply to every applicant.
