Case Manager - care planning (Houston, TX)

Lumen Vale Community Partners runs community programmes across Houston, and we need a Case Manager to lead our care planning. You would join a large organisation of more than 1,000 people based in Houston. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for benefits enrolment. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with benefits enrolment. Working arrangement • Contract for eight months, with clear deliverables from the first week. • On site in Houston. • We will tell you what to expect at each step of the hiring process.
