Case Manager - care planning (Houston, TX)

We are growing our team at Westbrook Amber Early Learning and adding a Case Manager whose main responsibility will be care planning. You would join a large organisation of about 333 people based in Houston. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for classroom supplies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with classroom supplies. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, spread across three days. • On site in Houston. • Applications are reviewed on a rolling basis.
