Case Manager - care planning (Houston, TX)

Foxglove Sable Community Partners runs community programmes across Houston. We are hiring a Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for grant reporting. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with grant reporting. Working arrangement • Full-time, with a steady weekday schedule. • On site in Houston. • We reply to every applicant.
