Case Manager - care planning (Honolulu, HI)

We are growing our team at Foxglove Civic Services and adding a Case Manager whose main responsibility will be care planning. You would join a small organisation of about 177 people based in Honolulu. What you will do • Help with applications and appointments for constituent requests. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with constituent requests. Working arrangement • Part-time, about 28 hours a week, afternoons. • On site in Honolulu, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
