Case Manager - care planning (Honolulu, HI)

We are growing our team at Stonegate Clear Care Services and adding a Case Manager whose main responsibility will be care planning. You would join a large organisation of about 871 people based in Honolulu. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for client records. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with client records. • Bilingual English and Spanish. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • On site in Honolulu. • Applications are reviewed on a rolling basis.
