Case Manager - care planning (Honolulu, HI)

Highbridge Timber Property Management looks after rental homes across the Honolulu area. We are hiring a Case Manager to take on care planning. You would join a large organisation of about 888 people based in Honolulu. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for rent collection. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with rent collection. Working arrangement • Part-time, about 28 hours a week, mornings. • On site in Honolulu. • We reply to every applicant.
