Case Manager - care planning (Honolulu, HI)

Nova Ember Community Partners is a nonprofit in Honolulu supporting families with food, housing and job help. We are hiring a Case Manager to take on care planning. You would join a small organisation of about 18 people based in Honolulu. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for donor records. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with donor records. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Honolulu. • We reply to every applicant.
