Case Manager - care planning (Honolulu, HI)

Bright Fern Care Services is a social-care provider serving older adults and people with disabilities around Honolulu, and we need a Case Manager to lead our care planning. You would join a large organisation of more than 1,000 people based in Honolulu. What you will do • Help with applications and appointments for licensing. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with licensing. • Bilingual English and Spanish. Working arrangement • Contract for six months, with the possibility of becoming permanent. • On site in Honolulu. • Applications are reviewed on a rolling basis.
