Case Manager - care planning (Hartford, CT)

Greenridge Drift Civic Services works with county and city offices around Hartford. We are hiring a Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for constituent requests. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with constituent requests. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in Hartford. • Applications are reviewed on a rolling basis.
