Case Manager - care planning (Hartford, CT)

Westbrook Glen Health is a healthcare provider in Hartford with clinics and support teams. We are hiring a Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for referrals. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with referrals. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, mornings. • On site in Hartford. • We reply to every applicant.
