Case Manager - care planning (Hartford, CT)

Ironwood Mill Care Services supports families and individuals in Hartford with personal care and coordination, and we need a Case Manager to lead our care planning. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for family communication. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Contract for twelve months, with the possibility of becoming permanent. • On site in Hartford. • Applications are reviewed on a rolling basis.
