Case Manager - care planning (Hartford, CT)

We are growing our team at Lumen Union Health Group and adding a Case Manager whose main responsibility will be care planning. You would join a small organisation of about 121 people based in Hartford. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for insurance authorisations. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with insurance authorisations. • Bilingual English and Spanish. Working arrangement • Contract for nine months, with the possibility of becoming permanent. • On site in Hartford. • We will tell you what to expect at each step of the hiring process.
