Case Manager - care planning (Hartford, CT)

Amberfield Harbor Care Services Partners supports families and individuals in Hartford with personal care and coordination. We are hiring a Case Manager to take on care planning. You would join a small organisation of about 42 people based in Hartford. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for client records. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with client records. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, mornings. • On site in Hartford. • We reply to every applicant.
