Case Manager - care planning (Hartford, CT)

Summit Kestrel Community Partners is a nonprofit in Hartford supporting families with food, housing and job help. We are hiring a Case Manager to take on care planning. You would join a large organisation of about 900 people based in Hartford. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for benefits enrolment. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with benefits enrolment. Working arrangement • Part-time, about 20 hours a week, spread across three days. • On site in Hartford. • We will tell you what to expect at each step of the hiring process.
