Case Manager - care planning (Fresno, CA)

We are growing our team at Foxglove Loom Property Management and adding a Case Manager whose main responsibility will be care planning. Maintenance and leasing staff share information every day. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for vendor invoices. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with vendor invoices. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, with flexible days. • On site in Fresno. • We will tell you what to expect at each step of the hiring process.
