Case Manager - care planning (Fresno, CA)

Highbridge Basin Civic Services Group works with county and city offices around Fresno. We are hiring a Case Manager to take on care planning. You would join a small organisation of about 34 people based in Fresno. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for case files. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with case files. • Bilingual English and Spanish. Working arrangement • Contract for twelve months, with a review at the end and a chance of extension. • On site in Fresno. • We will tell you what to expect at each step of the hiring process.
