Case Manager - care planning (Fresno, CA)

Ironwood Maple Housing Group manages affordable housing communities across Fresno. We are hiring a Case Manager to take on care planning. Housing is our mission and residents are our neighbours. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for repair requests. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with repair requests. Working arrangement • Part-time, about 16 hours a week, spread across three days. • On site in Fresno, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
