Case Manager - care planning (Fresno, CA)

Clearpath Cross Civic Services Group is looking for a Case Manager. The job centres on care planning for permits, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for permits. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with permits. Working arrangement • Part-time, about 24 hours a week, spread across three days. • On site in Fresno, with some flexibility after the first few months. • We reply to every applicant.
