Case Manager - care planning (Fort Worth, TX)

Fernwood Lantern Community Partners Group is looking for a Case Manager. The job centres on care planning for grant reporting, and it would suit someone who likes steady, practical work. Flexibility and kindness are expected. What you will do • Help with applications and appointments for grant reporting. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with grant reporting. Working arrangement • Full-time, Monday to Friday. • On site in Fort Worth, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
