Case Manager - care planning (Fort Worth, TX)

Rivermark Vale Property Management Group is a Fort Worth property manager with leasing, maintenance and resident services. We are hiring a Case Manager to take on care planning. You would join a small organisation of about 44 people based in Fort Worth. What you will do • Help with applications and appointments for vendor invoices. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with vendor invoices. Working arrangement • Part-time, about 24 hours a week, afternoons. • On site in Fort Worth. • Applications are reviewed on a rolling basis.
