Case Manager - care planning (Fort Worth, TX)

Fernwood North Community Partners runs community programmes across Fort Worth. We are hiring a Case Manager to take on care planning. Mission matters here, and so does looking after staff. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for benefits enrolment. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with benefits enrolment. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Fort Worth. • We reply to every applicant.
