Case Manager - care planning (Fort Worth, TX)

Clearpath Ashton Health is a healthcare provider in Fort Worth with clinics and support teams. We are hiring a Case Manager to take on care planning. You would join a large organisation of about 318 people based in Fort Worth. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for clinic supplies. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with clinic supplies. Working arrangement • Contract for four months, with clear deliverables from the first week. • On site in Fort Worth, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
