Case Manager - care planning (El Paso, TX)

Silverline Stone Civic Services is looking for a Case Manager. The job centres on care planning for constituent requests, and it would suit someone who likes steady, practical work. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for constituent requests. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with constituent requests. • Bilingual English and Spanish.
