Case Manager - care planning (Detroit, MI)

Bluepeak Granite Housing is a nonprofit housing provider in Detroit. We are hiring a Case Manager to take on care planning. You would join a small organisation of about 48 people based in Detroit. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for repair requests. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with repair requests. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in Detroit. • Applications are reviewed on a rolling basis.
