Case Manager - care planning (Detroit, MI)

We are growing our team at Westbrook Maple Housing and adding a Case Manager whose main responsibility will be care planning. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for rent assistance. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with rent assistance. Working arrangement • Full-time, with a steady weekday schedule. • On site in Detroit. • Applications are reviewed on a rolling basis.
