Case Manager - care planning (Detroit, MI)

Foxglove Thistle Learning Group is an education provider in Detroit serving students and families. We are hiring a Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for family communication. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Contract for six months, with clear deliverables from the first week. • On site in Detroit. • We reply to every applicant.
