Case Manager - care planning (Detroit, MI)

We are growing our team at Amberfield North Early Learning and adding a Case Manager whose main responsibility will be care planning. You would join a small organisation of about 106 people based in Detroit. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for licensing records. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with licensing records. Working arrangement • Part-time, about 20 hours a week, afternoons. • On site in Detroit. • We will tell you what to expect at each step of the hiring process.
