Case Manager - care planning (Des Moines, IA)

We are growing our team at Bluepeak Linden Health and adding a Case Manager whose main responsibility will be care planning. Clinicians and administrators work side by side. What you will do • Help with applications and appointments for patient scheduling. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with patient scheduling. Working arrangement • Contract for six months, with the possibility of becoming permanent. • On site in Des Moines. • We will tell you what to expect at each step of the hiring process.
