Case Manager - care planning (Des Moines, IA)

We are growing our team at Rivermark Vista Property Management and adding a Case Manager whose main responsibility will be care planning. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for lease renewals. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with lease renewals. Working arrangement • Full-time, with a steady weekday schedule. • On site in Des Moines, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
