Case Manager - care planning (Des Moines, IA)

Stonegate Haven Early Learning is looking for a Case Manager. The job centres on care planning for family communication, and it would suit someone who likes steady, practical work. You would join a large organisation of more than 1,000 people based in Des Moines. What you will do • Help with applications and appointments for family communication. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • On site in Des Moines. • Applications are reviewed on a rolling basis.
