Case Manager - care planning (Des Moines, IA)

Harborlight Summit Housing provides affordable homes and support services around Des Moines, and we need a Case Manager to lead our care planning. You would join a large organisation of about 228 people based in Des Moines. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for resident services. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with resident services. Working arrangement • Contract for eight months, with a review at the end and a chance of extension. • On site in Des Moines. • Applications are reviewed on a rolling basis.
